NEMS’ Government and Community Affairs Team has been busy today meeting with our state Elected Officials and their staff up here in Sacramento for California Primary Care Association’s annual Day at the Capitol! Thanks to Assemblymember Phil Ting, Assemblymember Matt Haney, Assemblymember Alex Lee, Assemblymember Ash Kalra, Senator Scott Wiener, Senator Aisha Wahab, MBA, and Senator Dave Cortese for representing our staff, patients, and 23 sites, which serve over 72,000 patients in the San Francisco Bay Area! It was great to see our sister FQHCs out here in full force advocating for a better healthcare future for all. Let’s keep the momentum going! 🙌🏼 #DAC2024

Camden Coalition has a bit of a cult-like status within complex care models, and the work there is ground-breaking, mission-oriented and thoughtful. Given my admiration for them, the studies showing their lack of demonstrable results point to a simple fact:Care coordination alone, even when implemented well, is insufficient to reduce hospitalizations for this [really, any] complex population of high-need patients.Primary care has proven its value. I am more concerned that a lot of the innovative, Specialty-focused models seem to rely on navigation, coordination and peer supports - without learning the lessons that are tried and true about complex populations. The reality is that these populations actually need a place to go when their needs escalate, a primary care provider to help them and real specialist access/integration for their needs. Let's hope that the next wave of these innovative companies learn these old lessons - these populations deserve it.https://lnkd.in/ecsjcbY2

💚 Today, Care England has launched the #AprilCareAgenda, a newsletter tailored for care providers. To begin this edition, Chief Executive Prof Martin Green shares his insight:🗣 "The Care England conference, Championing Change, was a good opportunity for Care England members to come together and debate the challenges and opportunities of the future. It is quite clear that we cannot rely on politicians coming to our aid, and so increasingly, we have got to think strategically about how we craft our destiny and take control of our future."📨 Read his full quote, along with articles from care sector leaders - in your inbox now!

We're on the lookout for trailblazers ready to tackle anti-Black racism within our state's health care delivery system. We're thrilled to announce a new Request for Proposals (RFP) for quality improvement projects dedicated to dismantling systemic barriers and enhancing equitable medical care for Black Californians.🌟 Grant Opportunity: Up to $150,000 for a two-year period. 🎯 Focus: Implementing and assessing impactful strategies in public or private health settings. ⏳ Deadline: January 31, 2024.CHCF encourages smaller grants that assess small tests of change or that lay essential groundwork for assessing equitable care. These efforts are often more likely to be adopted in regular operations. The goal is to fund a mix of smaller and larger efforts that can be integrated into regular operations. This is more than a grant – it's an opportunity to be at the forefront of meaningful change in our communities.Learn more.https://lnkd.in/gvUv_aTu#HealthEquity #RFP #QualityImprovement #AntiRacism #HealthCareInnovation
