For all the Epic EHR professionals out there, here is one for the books.I went to the doctor todayThe doctor said he couldn't see my medical recordsI said, " you are on the same.platform, can't you ask them to give you permission?" (Both groups use Epic's MyChart.)He said, "Most doctors don't know how to do that."I have to fax a request for the paper copies to be obtained.😑Last I checked it is 202_ AD.

    Ever wondered why Epic’s EHR is so strong in the USA ?The EHR market is super fragmented in Europe but not in the US.I just learned some that will surely interest you all!Here’s a summary:Under Obama, the HITECH Act was passed to increase adoption of EHR sponsoring doctors to buy first and then to use it on top of that!1. Giving them up to 44k$ to buy an EHR2. Paying them a further 17k$ if they proved they used it (for meaningful use)Now that favours EPIC by increasing the market size as a whole but now looking at the details:The feature set required for getting the subsidy looks very similar to EPIC and the department of Justice prosecuted the competitors of EPIC because they did not have them all!Then during COVID:1. the USA selected 3 providers (Abbott, Ellume and Quidel)2.     Lots of people sharing this viral talk about regulatory capture in technology.Jason Pettiaux points out the specific problem with regulatory capture in healthcare technology (and healthcare more broadly). He notes the non-market determined pricing power that these government manipulating corporate practices enable. I would add that with regulatory capture you not only do you get a higher priced product, you get a crappier one. When my friends in tech ask me why healthcare is stuck in the 1990s with its technology, I have one answer: Epic and it’s regulatory capture of health ITThe biggest irony is now new LLM technology is going to be very difficult to apply and train in US hospitals because Epic’s “turn doctors into medical billing data entry clerks” style of click-medicine has generated 15 years of utterly useless notes from a LLM training perspective. Unlike handwritten or dictated notes, EMR generated click box notes capture little of the actual thinking that led to the medical decision making that LLMs need to be trained on at scale. 🤦♂️#healthtech #healthcareinnovation #healthcare

    I think this raises a great reienforcing feedback loop for the rise of Epic in the US, but I am convinced their focus on financial capture (look at their KLAS scores in those areas) has put them as a necessity in the US and organizations will tolerate sub optimal if not near innoperable clinical build for that financial capture. IDNs/facilities in countries that don’t have to deal with revenues capture are not as dependent on that highest performing area of Epic’s IS and allows for the healthier mix of clinical capability and operations support seen in other EHRs

    One question the Change Healthcare fiasco raises - do we really need a revenue cycle management flow that goes through so many steps? Today, could we move to a system where patients just pay an estimated cost at the point of care? But to understand how this works, you need to understand how a set of services gets turned into a bill which turns into cash for a doctor.
